Supra-malleolar Orthoses (SMOs)

SMOs help children move more comfortably by guiding the foot into a more efficient position for walking. When the heel is properly aligned, the rest of the leg can work more smoothly, making steps feel easier and less tiring. Many parents notice their child walks for longer and with more confidence once the foot is better supported. SMOs don’t restrict movement—they simply offer stability where it’s needed.

For children whose ankles roll inward or who struggle with balance, SMOs provide extra support around the heel and ankle. This helps prevent collapsing through the foot and gives your child a steadier foundation. Improved stability can make a big difference when children are learning to stand, step or keep up with others during play. Over time, this support may help build confidence and independence.

Feet that roll in or collapse can place extra strain on the muscles and ligaments of the foot and lower leg. SMOs can help reduce this discomfort by improving alignment and evenly distributing pressure. Children who were previously sore or fatigued may feel more comfortable walking and playing. SMOs don’t treat the underlying cause of pain, but they can help make day-to-day movement more manageable.

Without support, ongoing pronation or ankle collapse can lead to tight muscles, foot deformity or long-term alignment issues. SMOs help guide the foot into a healthier position during growth, reducing the risk of future problems. They provide enough control to support development while still allowing natural movement. Regular reviews help ensure the right level of support as your child grows stronger.

Referrals aren’t necessary for a number of appointments. If you’d like your child assessed for head shape, toe walking or mobility, you can simply book an appointment online or by phone on 03 9836 4480.

Some conditions do need a referral however. For conditions such as hip dysplasia, scoliosis or pectus, or for management of fractures or acute injury, you’ll need a referral from your GP or other medical specialist.
