Referral Terms & Conditions

By submitting a referral through the Orthokids website, you confirm and agree to the following:

1. Authority to Refer

You confirm that you are authorised to make this referral and are acting within your professional capacity (where applicable).

2. Consent to Share Information

You confirm that you have obtained informed consent from the child’s parent or legal guardian to collect and disclose the child’s personal information and sensitive health information to Orthokids for the purpose of assessment, treatment, and service coordination.

You acknowledge that it is your responsibility to ensure that consent has been properly obtained in accordance with applicable privacy laws.

3. Privacy and Handling of Health Information

Orthokids collects, uses, stores and discloses personal information and sensitive health information in accordance with:

• The Privacy Act 1988 (Cth)
• The Australian Privacy Principles (APPs)
• Any applicable State or Territory health records legislation
• By submitting a referral, you acknowledge and agree that:
  • Personal and health information provided will be collected for the primary purpose of assessing and providing orthotic and related allied health services.
  • Information may be disclosed to treating practitioners, support coordinators, funding bodies (e.g. NDIS), Medicare, private health insurers, or other relevant third parties where required for the provision of care or administration of services.
  • Information will be securely stored and managed in accordance with Orthokids’ Privacy Policy.
  • Individuals may request access to or correction of their personal information in accordance with applicable law.

4. Accuracy of Information

You agree that all information provided in the referral form is accurate, current, and complete to the best of your knowledge. Orthokids is not responsible for delays, service limitations, or clinical outcomes resulting from incomplete, inaccurate, or misleading information.

5. Clinical Review and Acceptance

Submission of a referral does not guarantee acceptance for services. All referrals are subject to clinical review, practitioner availability, and service capacity.

6. Communication

You consent to Orthokids contacting you and/or the child’s parent or legal guardian to:

• Obtain additional information
• Clarify referral details
• Facilitate appointment scheduling
• Coordinate care with relevant providers

7. Website Use

You agree not to misuse the online referral system, submit false or unauthorised referrals, or attempt to interfere with the operation or security of the website.
